Notice of Privacy Practices (NPP)

This mandatory document explains your rights regarding your Protected Health Information (PHI), how we use and protect your records, and under what specific circumstances information may be disclosed (e.g., for treatment, payment, or healthcare operations).

Informed Consent for Telehealth Services

This is the core treatment agreement. It outlines the therapeutic process, defines the risks and benefits of virtual therapy, details our confidentiality rules, and explains essential policies like the 48-hour cancellation rule and emergency protocol.

Good Faith Estimate (GFE)

Required by the No Surprises Act, this document provides an estimate of the total costs you can expect to pay for services if you are uninsured or choose to pay out-of-pocket (self-pay)

Emergency Protocol and Crisis Contact Form

This critical safety plan provides 24/7 resources and contact information for immediate mental health crises (such as 988, 911, and local hotlines). It clarifies that Feel Wright Space is not an emergency service.
